Esther Eleboh: Comparison of IVF Ovarian Stimulation Protocols
Esther Eleboh, Reproductive Health Nurse (Head of Nursing Department) at Viva Fertility Clinic, shared a post on LinkedIn:
“Premature Luteinization
One of the challenges we encounter during controlled ovarian stimulation in IVF is premature luteinization, particularly when managing patients with a high ovarian response.
The goal is not just to recruit multiple follicles but also to maintain optimal hormonal conditions for effective cycle management.
Where does Progestin-Primed Ovarian Stimulation (PPOS) fit in? Research suggests that PPOS can be an effective alternative for selected patients, particularly when a freeze-all strategy is planned.
By using progestins to suppress the premature LH surge, PPOS offers another approach to preventing premature ovulation during ovarian stimulation.
Why consider PPOS over a GnRH agonist protocol in selected patients?
- Effective suppression of premature LH surges.
- An alternative approach to pituitary suppression during ovarian stimulation.
- Suitable for treatment cycles where all embryos will be frozen for subsequent frozen embryo transfer (FET).
- An opportunity to individualize stimulation protocols according to the patient’s ovarian response and treatment goals.
Protocol selection should consider the patient’s ovarian reserve, previous response to stimulation, risk of ovarian hyperstimulation syndrome (OHSS), treatment objectives, and available evidence.
One important consideration: PPOS generally requires a freeze-all approach because the progestin used during stimulation makes the endometrium unsuitable for a fresh embryo transfer.
My takeaway: Successful IVF is not about using one protocol for every patient. It is about understanding the patient’s physiology and selecting the most appropriate protocol for that individual.
I would love to hear your clinical insights and experiences.”
