Lucile Ferreux: Paternal Age as an Underestimated Factor in Reproductive Health

Lucile Ferreux, Medical Director at Cesar Fertility, shared a post on LinkedIn about a paper by Chee Wai Ku et al. published in Fertility and Sterility:

“Paternal age: a still underestimated factor in reproductive health

It is still wrongly thought that male fertility is not very sensitive to time.
However, scientific publications show that sperm quality changes with the advancement of paternal age.

In particular, we observe:

an alteration of certain sperm parameters,
an increase in sperm DNA fragmentation,
an accumulation of de novo genetic mutations,
epigenetic modifications that can influence embryonic development and the health of offspring.

These changes are associated, according to studies, with an increased risk of infertility, miscarriage and certain long-term effects in children among men over 45 years of age.

A recent article (Paternal Age and Reproductive Planning, FertilSteril 2025) puts this data back at the center of preconception thinking and highlights the lack of information for men and couples about the potential impact of age.

The authors discuss, in some contexts, different reproductive planning options, including sperm freezing as a preventive measure.
This study underlines that these approaches must be discussed in the light of the cultures, ethical frameworks and health systems specific to each country, which are very different from the French model.

Beyond the technical options, the main challenge remains the integration of male reproductive aging within the framework of accurate preconception information without excessive medicalization or normative message.

How, in your opinion, can we better inform about paternal age and its biological effects in our clinical practices, while respecting the cultural and organizational specificities of our healthcare systems?”
