“AJOG Expert Review: Immune tolerance breakdown in pregnancy: transplantation parallels in chronic intervillositis and villitis of unknown etiology – Schematic representation of immune dysregulation in villitis of unknown etiology (VUE)
The decidua and intervillous space exhibit a dominant infiltration of maternal CD8+ T cells, expressing the chemokine receptor CXCR3, alongside scattered histiocytes and CD4+ T cells. These CD8+ T cells target chorionic villi abnormally expressing polymorphic HLA molecules. The villous stroma is infiltrated by CD8+ T cells causing structural damage. Fetal Hofbauer cells become activated and express HLA class II molecules. The fetal endothelium also shows aberrant expression of polymorphic HLA. These findings are consistent with a T cell–mediated rejection (TCMR) phenotype.
