“A ‘normal’ semen analysis does not tell you the whole fertility story.

A standard semen analysis looks at important basics: sperm concentration, total number, motility, morphology and semen volume.

But sperm health is more than a headcount and a movement score.

In selected cases, especially with recurrent pregnancy loss, unexplained infertility or repeated IVF/ICSI difficulties, it may be worth asking whether further assessment is appropriate – including sperm DNA fragmentation.

And then there’s the bigger picture.

Sperm are affected by the environment they’re produced in: metabolic health, smoking, alcohol, recent illness or fever, heat exposure, medications, varicocele, diet, sleep and other health factors can all matter.

Sperm production also takes roughly 2½–3 months, so improving the underlying environment is not an overnight project.

Most importantly, fertility should not become a situation where she gets the blood tests, tracks ovulation, changes her diet, takes the supplements and carries the mental load while his ‘normal’ semen analysis closes the investigation.

Fertility is a couple’s biology. The assessment should reflect that.
