Devyanshi Dixit: Why Does Stress Sometimes Amplify Gynecological Pain?
Devyanshi Dixit, Program Manager at CLS Event and Publication and Conference Manager at the 12th Global Summit on Gynecology, Fertility and Women’s Health, shared on LinkedIn:
“Stress can amplify pelvic pain. That does not make the pain psychological – or imaginary.
The biology is more interesting than the usual explanation.
Pain is not a direct readout of tissue damage.
It emerges from signals travelling from the body and how the nervous system processes them. In some people with persistent gynecological pain, the system that normally detects and regulates pain can become more sensitive.
This is known as central sensitization.
Stress, poor sleep and sustained vigilance can interact with pain-processing pathways and make symptoms harder to regulate.
Pain itself can then increase stress, creating a feedback loop.
In conditions such as endometriosis, researchers are investigating how inflammation, nervous-system changes and stress responses may reinforce one another.
But an important distinction matters: a mechanism that can amplify pain is not necessarily the original cause of it.
Stress does not explain every case of pelvic pain, and its presence should never replace an assessment for conditions such as endometriosis, adenomyosis, pelvic-floor dysfunction or other gynecological and non-gynecological causes.
Nor does a treatment plan that includes psychological support mean the pain is ‘all in the head.’
Pain education, pelvic-floor physiotherapy and psychological approaches may complement medical or surgical care – not invalidate it.
The clinical challenge is to assess the source of pain while also understanding the systems that may be sustaining it.
That is a more useful question than asking whether pain is physical or psychological.
How can clinicians discuss stress-related pain amplification in a way that validates patients’ experiences while still investigating the underlying pathology?
