“Dr. Raoul Palmer was a French gynecologist who, according to many, made a decisive contribution to certain surgical aspects of modern laparoscopy․
- Palmer’s point entry is an alternative surgical site in the left upper quadrant used for Veress needle or optical trocar insertion during gynecologic laparoscopy when umbilical entry is unsafe․
- Position: located 2 to 3 cm below the left costal margin in the midclavicular line․
- Visceral and parietal adhesions are rare in this upper quadrant area compared to the periumbilical region․
What about the most important indication procedure?
- Umbilical abnormalities, such as umbilical hernia, infection, or previous mesh repair․
- Large pelvic or uterine masses (such as large fibroids or ovarian cysts) reaching above the umbilicus․
- Extremely thin patients where major vascular bifurcation sits very close to the anterior wall, or after failed umbilical entry attempts․
The Veress needle or optical trocar is typically inserted at a 90-degree angle to the abdominal wall regardless of the patient’s BMI․
