“When ‘No Doppler Flow’ Does Not Mean ‘No Ectopic Pregnancy’

A positive pregnancy test + amenorrhea + pelvic pain/bleeding + no definite intrauterine pregnancy should trigger a systematic adnexal assessment.

Technical ultrasound hints:

  • Don’t rely on the ‘ring of fire’ alone.
  • An ectopic gestation may be surrounded by hematoma/organized clot, which can appear solid and avascular.
  • Absence of Doppler vascularity does not exclude ectopic pregnancy.
  • Always assess for hemoperitoneum, including Morrison’s pouch.
  • Correlate with β-hCG, clinical findings, and serial ultrasound when appropriate.

The key is not one sonographic sign – it is the pattern.”
