Esther Eleboh: When Euploid Does Not Always Mean Competent

“When Euploid Does Not Always Mean Competent.

A recent case reminded us of an important lesson in embryo selection and PGT-A.

A 35-year-old woman underwent PGT-A after a good ovarian stimulation:

  • 28 oocytes retrieved
  • 23 MII
  • 22 × 2PN
  • 5 MI/GV
  • 7 embryos biopsied on Day 5
  • 5 embryos were Grade A
  • 2 embryos were poor-quality

The PGT-A results were quite interesting.

Among the 5 good-quality Grade A embryos, only 1 was euploid, while the remaining 4 were aneuploid.
Surprisingly, both poor-quality embryos were euploid.

The two euploid poor-quality embryos were subsequently transferred into a good endometrium, but unfortunately, the pregnancy test was negative.

What does this teach us?

Euploidy is extremely important – but it is not the only factor that determines implantation and pregnancy. Embryo morphology and developmental competence still matter.

A genetically normal embryo may not necessarily have the developmental potential required to implant and establish an ongoing pregnancy.

This case highlights why we should avoid looking at PGT-A results in isolation.

Embryo competence = genetics + morphology + developmental potential + endometrial receptivity + other biological factors.

PGT-A tells us about chromosome status.

It does not guarantee implantation or live birth.

Every embryo deserves careful consideration, and every clinical outcome should be interpreted within the bigger picture.
