“Dr. Raoul Palmer was a French gynecologist who, according to many, made a decisive contribution to certain surgical aspects of modern laparoscopy․

  • Palmer’s point entry is an alternative surgical site in the left upper quadrant used for Veress needle or optical trocar insertion during gynecologic laparoscopy when umbilical entry is unsafe․
  • Position: located 2 to 3 cm below the left costal margin in the midclavicular line․
  • Visceral and parietal adhesions are rare in this upper quadrant area compared to the periumbilical region․

What about the most important indication procedure?

  1. Umbilical abnormalities, such as umbilical hernia, infection, or previous mesh repair․
  2. Large pelvic or uterine masses (such as large fibroids or ovarian cysts) reaching above the umbilicus․
  3. Extremely thin patients where major vascular bifurcation sits very close to the anterior wall, or after failed umbilical entry attempts․

The Veress needle or optical trocar is typically inserted at a 90-degree angle to the abdominal wall regardless of the patient’s BMI․
