Here are some key reasons for this fluctuation:

  1. Shifting Diagnostic Criteria: The prevalence can be 12.1% using the Rotterdam criteria but drops to 7.9% under the stricter NIH criteria.
  2. Inconsistent Symptom Definitions: Different studies use varying thresholds for symptoms like hirsutism, leading to different diagnosis rates.
  3. Inaccurate Administrative Databases: Health records often show low rates (0.13% to 5.2%) due to misclassification and lack of access to specialists.
  4. Genuine Regional Differences: Adult rates are highest in the Eastern Mediterranean (15.1%) and South-East Asia (14.3%), compared to 10.5% in the Americas.
