“A semen report can come back completely normal, and the couple still cannot conceive.

That single sentence took up most of the session last week.

A room of gynaecologists, in from across the country, spent the day at the microscope with me. Semen analysis read properly, not just the top-line count. Sperm processing across multiple techniques. Vitality. Morphology under strict criteria. And sperm DNA fragmentation, which is where the conversation kept returning.

Fragmentation is the one that catches people. The count is fine, motility is fine, the report looks clean, and the DNA inside those sperm is damaged. It sits quietly behind a lot of unexplained infertility and recurrent pregnancy loss in this country, and it is still rarely ordered.

Not because anyone is careless. Because andrology is barely taught, and you cannot order a test you were never shown.
