“Every IVF cycle carries a hidden question: will this patient’s ovaries overreact?

Ovarian Hyperstimulation Syndrome (OHSS) remains one of the most preventable – yet still underestimated – complications in assisted reproduction. The good news: risk is rarely a surprise. It’s usually predictable, if we look at the right numbers early enough.

A well-built OHSS risk calculator brings together the variables we already collect during stimulation:

  • Antral follicle count (AFC)
  • Baseline and peak AMH
  • Estradiol level on trigger day
  • Number of follicles> 14 mm
  • PCOS status and BMI
  • Total oocytes retrieved

Instead of relying on gut feeling at trigger day, a structured scoring model flags high-risk patients earlier – often early enough to shift strategy: switching to a GnRH agonist trigger, adopting a freeze-all approach, or starting cabergoline prophylaxis before symptoms even begin.

This isn’t about replacing clinical judgment. It’s about giving it better inputs. A five-second risk score at the right moment can be the difference between a routine cycle and an emergency admission for a patient.
