Resa Nabor: What You Need to Know About PCOS and PCOM

Resa Nabor, Founder and Community Advocate of PCOS Millennials, shared on LinkedIn:

“PCOS and PCOM may sound similar, but they’re not the same. Learn the key differences and why understanding them matters for your health.

‘Hearing you have ‘cysts’ on your ovaries makes people instantly assume they’re sick.

But the syndrome PCOS is very different from the physical trait PCOM.

Take a healthy 23-year-old with regular periods whose routine ultrasound shows ‘polycystic’ ovaries.

Those dark spots aren’t actually harmful cysts at all. They’re just a high number of tiny, immature egg follicles packed inside.

This physical structure is called PCOM – Polycystic Ovarian Morphology.

It’s just a visual description, and it’s incredibly common in healthy young women.

But here’s the problem: an ovary with the actual disease looks exactly the same.

So how do doctors tell them apart?

It only becomes PCOS when a diagnostic checklist expands beyond the ovary.

You need system-wide hormonal symptoms, like high androgens or irregular cycles.

To be sure, doctors test the chemical signal these tiny follicles produce.

That signal is called Anti-Müllerian or AMH.

Measuring AMH levels in the blood helps doctors separate a harmless visual trait from clinical PCOS.

So for our healthy 23-year-old, her checklist stops at the ultrasound.

She has PCOM, not PCOS – meaning her ‘polycystic’ ovaries are actually completely normal.’
