”It takes twelve years to become an OB-GYN: four years of college, four of medical school, four of residency. More if you subspecialize.

Then the real education begins.

Thirty-five years later, the most important thing experience has taught me is how much I don’t know. When a patient asks a hard question, my honest answer often starts with ‘it depends.’ That isn’t evasion. It’s medicine.

Nuance is at the heart of clinical care, and you can’t download it. It’s earned at the bedside, one patient at a time, especially the ones who didn’t follow the textbook.

Online, the opposite gets rewarded. Certainty spreads, and fear spreads faster. Someone reads a study for an hour, misses its limitations, and announces a conclusion the authors never claimed.

Sometimes it’s a layperson. Sometimes it’s a credentialed professional far outside their training. Sometimes, honestly, it’s a physician outside their specialty.

The title ‘doctor’ isn’t the problem. Scope is. Knowing the vocabulary doesn’t make you an expert in the field.

The person who pays is a pregnant woman at 2 a.m., scrolling, trying to decide whom to believe.

And it’s about to get harder. AI can now produce a confident answer to almost anything in seconds, complete with citations. Confident answers have never been cheaper.

Nuance has never been worth more.
