“It All Comes Down to One Molecule

OHSS can feel like a mystery — but the swelling really does trace back to a single signal called VEGF. Here’s what it does, why the gentler trigger usually keeps it in check, and why a small group of strong responders need a closer eye. Knowledge is what lets us stay one step ahead.

So much of what happens in IVF can feel invisible — but the swelling some women experience after egg collection comes down to one molecule you can actually point to. It’s called VEGF (vascular endothelial growth factor).

Here’s how it works: After your eggs are collected, the granulosa cells release VEGF, which opens up tiny blood vessels nearby. In small amounts, that’s completely normal. Trouble starts when there’s too much of it.

This is where the trigger injection matters! The newer GnRH agonist trigger gives only a short surge, keeping VEGF low and vessels sealed. Most women sail through because of this.

But remember, the more follicles your ovaries hold, the more granulosa cells are releasing VEGF all at once. Past a certain point, the vessels leak enough fluid into the abdomen to cause OHSS. This is why strong responders are monitored closely.

The good news? Understanding the cause lets us stay ahead of it! If your follicle count is high, we plan a freeze-all and monitor you closely. We act before anything starts, not after.
