Christopher Robinson, Associate Professor Maternal Fetal Medicine at University of South Carolina, shared a post on LinkedIn:
“AJOG Expert Review: Immune tolerance breakdown in pregnancy: transplantation parallels in chronic intervillositis and villitis of unknown etiology – Schematic representation of the healthy fetal–maternal interface
Three distinct compartments are illustrated: the maternal decidua, the intervillous space, and the fetal chorionic villus. The decidua is traversed by a spiral uterine artery and composed of stromal cells providing structural support, along with maternal immune cells, mainly immunoregulatory CD56+ uterine NK cells, FOXP3+ regulatory T cells, and decidual macrophages, which contribute to a tolerogenic environment for the fetus. The spiral artery opens into the intervillous space, where a floating chorionic villus is bathed in maternal blood.
From the outside inwards, the villus comprises a syncytiotrophoblast (SCT) with an apical brush border, a cytotrophoblast layer, villous mesenchyme, and fetal capillaries. Fetal Hofbauer macrophages reside in the mesenchyme. The SCT expresses immunomodulatory molecules, including PD-L1 and complement regulatory proteins (CD55/CD59). It also releases extracellular vesicles with systemic immunoregulatory properties.”
