“I am pleased to share our new systematic review published in the European Journal of Medical Research, examining a critical and often debated topic in obstetric practice:
the impact of progesterone supplementation on Doppler ultrasound indices and maternal–perinatal outcomes.
Across nine studies comprising 2,282 pregnant women, our review provides one of the most up-to-date syntheses on how progesterone affects uteroplacental and fetal hemodynamics.
Key Findings
Our analysis revealed that progesterone supplementation demonstrates inconsistent effects on Doppler ultrasound parameters. While some studies reported reductions in uterine artery resistance, most showed no significant changes in uterine or umbilical artery Doppler indices.
However, isolated evidence indicated a decrease in fetal middle cerebral artery pulsatility index, suggesting a possible vasodilatory effect on fetal circulation that warrants further investigation.
Clinically, progesterone did not significantly improve rates of IUGR, preeclampsia, or PROM across the included studies.
Yet, consistent with prior literature, certain high-risk subgroups—especially women with a short cervix—experienced reduced risk of preterm birth.
Why This Matters
Progesterone use in pregnancy remains widespread, often without uniform guidelines. Our findings highlight the need for:
Individualized clinical decision-making, rather than routine progesterone prescription.
Standardized progesterone dosing regimens and unified Doppler outcome definitions to reduce heterogeneity in future research.
Recognition that Doppler changes alone should not drive progesterone therapy, as hemodynamic effects are not consistently demonstrated.
Overall, this review underscores progesterone’s targeted benefits while calling for more rigorous evidence to refine its role in pregnancy management.
