Globozoospermia is a rare form of teratozoospermia in which spermatozoa have a round-headed appearance and lack a normal acrosome. Because the acrosome contains enzymes required for normal interaction with and penetration of the oocyte, fertilization can be severely impaired. It is also called round-headed sperm syndrome.The major problem occurs during spermiogenesis, particularly during formation and attachment of the acrosome to the sperm nucleus.
Normally:
Golgi apparatus – proacrosomal vesicles – acrosomal vesicle – acrosome spreads over anterior nucleus – nucleus elongates – mature sperm
In globozoospermia, this process is disturbed. Result: Abnormal acrosome formation – failure of nuclear elongation – round sperm head – abnormal sperm-oocyte interaction
Type I – Complete globozoospermia
- Almost/all sperm have round heads.
- Acrosome is absent.
- Usually associated with severe fertilization problems.
Type II – Partial globozoospermia
- Only a proportion of sperm are round-headed.
- Other sperm may have varying degrees of head/acrosomal abnormalities.
- Fertilization potential can be better than in complete globozoospermia, depending on the sperm population.
Normally, sperm undergo:
Capacitation – hyperactivation – binding to zona pellucida – acrosome reaction – interaction/penetration – oolemma fusion – oocyte activation
In globozoospermia, several steps can be impaired: absence of acrosome, Abnormal sperm-oocyte interaction, Poor oocyte activation, Abnormal sperm head structure
For an embryologist, the important workflow is:
- Semen sample
- Assess morphology
- Identify round-headed sperm / suspected globozoospermia
- Assess motility and vitality
- ICSI usually preferred in severe cases
- Careful assessment of oocyte activation
- Check fertilization at appropriate time point
- 2PN + 2PB – normal fertilization
Key point to remember:
Globozoospermia is primarily a sperm-head/acrosome disorder, but its major clinical consequence is impaired fertilization.
Globozoospermia – round sperm head – absent/abnormal acrosome – impaired sperm–oocyte interaction + possible impaired oocyte activation – very low fertilization – ICSI commonly used.
