“In PGT-A, how many trophectoderm cells should we really be taking at blastocyst biopsy? A recent study in Reproductive BioMedicine Online challenges the assumption that ‘bigger’ biopsies lead to better outcomes.
The findings suggest that diagnostic failure rates remain comparable regardless of the number of cells removed, indicating that less may actually be more! With diagnostic efficiency rising steeply between 1 and 5 cells (up to 97–99%), larger biopsies (around 10 cells) can reduce implantation rates, especially in embryos with poorer TE morphology.
For IVF programs, this means:
- Define target TE biopsy ranges (~5–8 cells) for consistency.
- Tailor biopsy size to TE grade and estimated TE cell mass.
- Incorporate biopsy metrics into lab auditing and training.
Let’s aim for standardized, evidence-based biopsy protocols that enhance diagnostic yield while preserving embryos. This not only boosts lab performance but also fosters patient trust in PGT-A.
