Mostafa Maghraby: Day 3 vs. Day 5 Embryo Transfer – Which Is Better?

Mostafa Maghraby, Embryologist, IVF and ICSI Specialist at Al-Amal Fertility Centre, shared on LinkedIn:

“Day 3 vs. Day 5 Embryo Transfer: Which Is Better?

One of the important decisions in IVF is determining when an embryo should be transferred.

Should we transfer an embryo on Day 3, at the cleavage stage, or continue culture until Day 5, when it may reach the blastocyst stage?

The answer is not always straightforward.

Day 3 Transfer – Cleavage Stage

At this stage, embryos are typically assessed based on cell number, blastomere symmetry, and the degree of fragmentation.

Potential advantages include:

  • Earlier transfer.
  • An option when extended culture may be challenging or when the number of available embryos is limited.

Day 5 Transfer – Blastocyst Stage

By Day 5, embryos that continue developing may reach the blastocyst stage.

Potential advantages include:

  • Additional information about developmental progression.
  • Better opportunities to differentiate embryos based on blastocyst morphology.
  • Potential for improved embryo selection in appropriately selected cases.

Does Day 5 always mean better outcomes?

No.

Extended culture does not guarantee a higher chance of live birth for every patient. Some embryos may stop developing before reaching the blastocyst stage, and the optimal strategy depends on the individual clinical situation.

Factors such as embryo number and development, patient history, laboratory performance, and clinical recommendations all matter.

The goal is not simply to reach Day 5.

The goal is to choose the strategy that best serves each patient’s circumstances.

Because in IVF, the best decision is not always the same for every case.”
